The Best AI Receptionist for Med Spas

A med spa loses bookings every time a consult call goes to voicemail. An AI receptionist can catch those calls, but it also hears health details, so the rules are stricter than for a hair salon. Here is what we would try first.

THE SHORT ANSWERUsable

Yes, an AI receptionist can now answer med spa calls, capture consult leads and hand callers to staff, but you need one that works with your booking software and will sign a HIPAA business associate agreement. If you book in Boulevard, start with its built in AI Receptionist at $125 a month per location. If you do not, My AI Front Desk is the most affordable option we found that says it signs BAAs.

The tools work for routine calls and lead capture today, but the best ones are tied to specific booking platforms, and HIPAA paperwork and clinical questions still need careful setup and human backup.

Boulevard AI Receptionist (Beau)

Best for med spas that already book in Boulevard and want calls tied to client profiles.

Boulevard's AI Receptionist, which the company calls Beau, lives inside the booking system many med spas already run. Boulevard says it already knows your service menu, hours, location details and policies, so you do not have to write a long script from scratch Boulevard AI Receptionist ↗. It answers 24/7, and you can run it all the time, only as a backup when the front desk misses a call, or only after hours Boulevard AI Receptionist ↗. It filters robocalls and spam Boulevard AI Receptionist ↗. For med spa leads it collects the caller's name, contact details and interest, such as an injectables consult, so staff can follow up Boulevard AI Receptionist ↗. Summaries and action items land in the same inbox your team already uses to text clients Boulevard AI Receptionist ↗.

Setup is short and built for clinics. You forward your main number to your Boulevard number, choose 24/7 or after hours coverage, and, for clinical businesses, add an emergency disclaimer to the greeting Boulevard setup guide ↗. You then choose whether the AI only takes messages or also transfers callers to a live person during business hours; after hours calls always go to messages Boulevard setup guide ↗. Boulevard asks you to keep the original caller ID when forwarding so callers can be matched to client profiles Boulevard setup guide ↗. You can listen to a sample call before you turn it on Boulevard setup guide ↗.

Pricing is published, which is rare in this category: $125 a month per location includes 200 minutes, with overage at $0.60 a minute, and Boulevard notes prices may change Boulevard AI Receptionist ↗. Boulevard lists HIPAA, SOC 2 and PCI Level 1 compliance on the page, but it does not say in plain words that these cover the AI's calls, so ask for that in writing Boulevard AI Receptionist ↗. One thing to check in a demo: the feature page says the AI sends a booking link when a caller is ready to book, while the setup guide says it books and manages appointments Boulevard AI Receptionist ↗ Boulevard setup guide ↗. Hands on testing is in progress; we will update this guide with results.

Why we like it

  • Built into Boulevard, so it knows your menu, hours and policies
  • Clinical emergency disclaimer is part of setup
  • Published price: $125 a month per location for 200 minutes
  • Choose 24/7, backup or after hours only coverage
  • Live transfer to staff during business hours

Flaws but not dealbreakers

  • Only for businesses that use Boulevard
  • Mixed wording on whether it books directly or sends a booking link
  • Supported languages are not stated
  • Few independent user reviews of the AI so far
Budget pickStarts at $99/month, or $79/month billed yearly

My AI Front Desk

Best for med spas on other booking software that want calls, texts and web chat with a signed BAA.

My AI Front Desk, which now also brands itself as Frontdesk, is the lowest cost tool we found that clearly says it is HIPAA compliant and signs business associate agreements, with encryption in transit and at rest My AI Front Desk features ↗. That matters for a med spa, because callers often mention medications, allergies or past treatments. When a call needs a person, it can warm transfer the caller or send you a real time alert with the full transcript My AI Front Desk features ↗.

The Business-in-a-Box plan is $99 a month, or $79 a month billed yearly My AI Front Desk pricing ↗. It includes 200 voice minutes, 400 text messages, 100 website chat conversations, a 20 page knowledge base, Zapier and two editor seats My AI Front Desk pricing ↗. Extra usage comes from prepaid credits: a voice minute costs 25 credits, which is $0.25 My AI Front Desk pricing ↗. There is a 7 day free trial My AI Front Desk pricing ↗. The company claims setup takes less than five minutes My AI Front Desk features ↗.

The weak spot for med spas is booking software. The site names connections for practice tools such as Jane App, SimplePractice and TherapyNotes, but it does not name Boulevard, Mindbody, Vagaro or Zenoti, so plan on Zapier or a manual handoff My AI Front Desk features ↗ My AI Front Desk pricing ↗. User reviews are split. On Trustpilot it scores 3.5 out of 5 from 15 reviews, with praise for easy setup and professional call handling and complaints about refunds, charges after trials and unanswered support emails Trustpilot, My AI Front Desk ↗. Start on monthly billing.

Why we like it

  • Says it signs BAAs and encrypts data
  • Calls, two way texts and web chat in one plan
  • Warm transfer or instant alert with transcript
  • 7 day free trial

Flaws but not dealbreakers

  • No named integration with common med spa booking platforms
  • Billing and refund complaints from users
  • Credit system takes a moment to understand

Zenoti AI Receptionist

Best for multi location med spas on Zenoti that want the AI to book, upsell and enforce policies.

Zenoti's AI Receptionist does more of the front desk job than any other tool here. It books single or multiple services while weighing provider preferences and schedules, suggests add on services and active offers during the call, and transfers to staff when a guest asks or the situation calls for it Zenoti AI Receptionist ↗. It can also call guests who have not confirmed a set number of hours before their visit, offer a live reschedule when someone cancels, and enforce cancellation fees automatically Zenoti AI Receptionist ↗.

It covers lunch, evenings, weekends and holidays, records every call with a summary and transcript, and reports revenue booked, calls handled and handover rate per location on a dashboard Zenoti AI Receptionist ↗. Setup does not require a new phone system: you forward calls to a Zenoti number Zenoti AI Receptionist ↗. Zenoti claims the AI turns about 1 in 3 missed calls into appointments and that 25 percent of those include an upsell; those are vendor numbers, not independent results Zenoti AI Receptionist ↗.

The catch is cost and commitment. You must be a Zenoti customer on its AI Plus package to use the AI agents, and Zenoti does not publish prices; you request a quote Zenoti pricing ↗. Voice and text usage is billed on consumption Zenoti pricing ↗. The product page does not mention HIPAA or list supported languages, so get both answered before you sign Zenoti AI Receptionist ↗. Zenoti launched the AI Receptionist as part of a wider set of AI agents in September 2025 Zenoti AI Workforce ↗.

Why we like it

  • Books directly in the Zenoti calendar, including multi service visits
  • Confirmation calls and automatic cancellation fees
  • Per location performance dashboard
  • Upsells add on services during the call

Flaws but not dealbreakers

  • Requires Zenoti and its AI Plus package
  • No public pricing
  • HIPAA and languages not addressed on the product page
Best free optionFree for 25 calls/month; paid plans start at $150/month

Smith.ai

Best for trying an AI receptionist on real calls before you commit, without patient health details.

Smith.ai has the only real free plan we found: 25 real calls a month at no cost, plus 25 test calls and 50 simulated calls so you can tune the script before callers hear it Smith.ai pricing ↗. After the free calls, extra calls cost $3.00 each Smith.ai pricing ↗. Paid plans start at $150 a month for 75 calls, and contracts are month to month with 30 days' notice Smith.ai pricing ↗. Known spam calls do not count toward your quota Smith.ai pricing ↗.

The AI handles answering, scheduling through Calendly and other tools, and payment collection, with native links to Clio and Zapier Smith.ai pricing ↗. It does not name Boulevard, Mindbody, Vagaro or other med spa platforms Smith.ai pricing ↗.

The big caveat for a med spa: the AI Receptionist pricing page does not mention HIPAA or a business associate agreement Smith.ai pricing ↗. Use the free plan to learn how an AI handles your calls, keep its script to hours, location, pricing ranges and consult requests, and do not let it collect treatment or health details until you have a signed BAA.

Why we like it

  • Free plan with 25 real calls a month
  • Test and simulated calls before going live
  • Month to month contracts

Flaws but not dealbreakers

  • HIPAA and BAA not mentioned on the pricing page
  • No named med spa booking integrations
  • Per call price is high once you pass the free tier

Prices and features from each vendor’s site, checked Oct 9, 2026. Prices change often.

Who this is for

This guide is for med spa owners and managers whose phones ring while the front desk is checking someone out, the injector is with a patient, or the doors are closed for the night. Med spa calls are often high value: a new client asking about lip filler or a laser package may be worth hundreds or thousands of dollars over a year. If that call goes to voicemail, the caller often books somewhere else.

An AI receptionist answers those calls, tells callers your hours and prices, collects consult requests, books or sends a booking link, and passes anything sensitive to a person. Boulevard's own blog says standalone AI receptionist tools can cost $500 or more a month depending on volume and features, so it pays to compare carefully Boulevard blog ↗.

If you have a full front desk team that answers nearly every call, you may only need after hours coverage. Most tools in this guide let you choose that. If you rarely miss calls, skip this purchase and keep doing what works.

Why you should trust us

We read each vendor's product, pricing and help center pages, checked every price on October 9, 2026, and looked for independent user reviews. We also read federal guidance on HIPAA business associate agreements HHS BAA guidance ↗ and a legal guide on call recording consent DMLP recording law ↗. Every fact in this guide links to the page it came from.

We have not tested these tools hands on yet. Hands on testing is in progress, and we will update this guide with results, including how each AI handles a consult request, a clinical question it should not answer, and a transfer to staff. No vendor paid for placement, and our picks were chosen on merit.

How we picked

We started with the AI receptionists that come up most for med spas: tools built into med spa booking platforms, general small business AI receptionists, and healthcare phone platforms. We dropped developer platforms and enterprise only contracts that a typical med spa cannot set up alone.

Two criteria mattered more for med spas than in our general small business guide. The first is the booking system. A receptionist that cannot see your real schedule can only take messages or send links, which loses some of the value. The second is HIPAA. Med spas offer medical treatments, and callers share health details, so the vendor should sign a business associate agreement before it handles that information Referrizer HIPAA article ↗. Where a vendor did not mention HIPAA on its pages, we said so.

  • Works with the booking software med spas actually use
  • Willing to sign a HIPAA business associate agreement
  • Transfers to staff for clinical or upset callers
  • Captures consult leads with name, contact and treatment interest
  • Published pricing, or at least a clear way to get a quote
  • Easy setup by a manager, not a developer
  • Independent user reports where they exist

What an AI receptionist can and can't do for a med spa today

It can reliably handle the routine calls that eat front desk time: hours, directions, parking, which treatments you offer, starting prices, and whether you have openings this week. Platform tools like Boulevard's and Zenoti's start with your service menu and schedule already loaded Boulevard AI Receptionist ↗ Zenoti AI Receptionist ↗. Zenoti's can also book multi service visits, make confirmation calls and offer a reschedule when someone cancels Zenoti AI Receptionist ↗.

It can capture leads well. Boulevard's AI records a caller's interest, such as an injectables consult, so a coordinator can call back Boulevard AI Receptionist ↗. Tools with texting, like My AI Front Desk, can follow up by text My AI Front Desk pricing ↗.

It should not give medical advice. Questions about whether a treatment is safe with a medication, what a reaction means, or whether someone is a good candidate belong with a licensed provider. Boulevard adds an emergency disclaimer step for clinical businesses during setup Boulevard setup guide ↗. Write your script so the AI says it will have a provider call back, and turn on transfers or alerts for anything urgent.

It is still weaker with upset callers, complex package questions and long conversations. Plan for a human backup, at least during business hours.

Before you sign up

Get the HIPAA paperwork first. HHS describes a business associate agreement as a written contract that sets how a vendor may use and disclose protected health information and requires safeguards for it HHS BAA guidance ↗. The rules generally require one when an outside company handles that information for you HHS BAA guidance ↗. Without a signed BAA, assume a service is not appropriate for protected health information Referrizer HIPAA article ↗. Upfirst, for example, lists BAAs only on its Custom plan Upfirst pricing ↗.

Know your call recording law. Federal law needs only one party's consent to record, but states including California, Florida, Illinois, Massachusetts, Pennsylvania and Washington require consent from everyone on the call DMLP recording law ↗. A short line in your greeting, such as "this call may be recorded," covers most cases.

Ask what happens to your data. Good questions include where recordings are stored, how long they are kept, whether your calls train the vendor's AI models, and what happens to the data if you cancel Referrizer HIPAA article ↗.

  • Get a signed BAA before the AI hears patient health details
  • Add a recording notice and an emergency disclaimer to the greeting
  • Keep the original caller ID when you forward calls
  • Script the AI to defer all medical questions to a provider
  • Set a transfer number that is not your main line
  • Start on monthly billing and review call transcripts every week

What to look forward to

Booking platforms are adding more AI jobs around the receptionist. Zenoti has announced an AI Lead Manager and AI Marketer as coming soon, and an AI Scribe for provider notes as planned Zenoti AI Workforce ↗. Weave says real time language switching for its AI receptionist is coming soon Weave AI Receptionist ↗.

We expect more platforms to publish prices and to state clearly whether their AI calls are covered by a BAA. We will update this guide as that happens and as our hands on testing finishes.

The competition

Podium AI EmployeeQuote only (add on to a Podium plan)

Podium's AI Employee covers calls, texts, email, social and web chat from one inbox, books on your calendar, sends reminders and reaches out to past clients, and it features a med spa owner testimonial Podium AI Employee ↗. Prices are not published and the AI Employee is an add on to a Podium plan, so we could not compare its value Podium pricing ↗.

Weave AI ReceptionistQuote only

Weave's AI Receptionist answers calls and texts 24/7 and says it is designed to meet HIPAA standards, but automatic booking is only for qualified dental practice software users and the page does not mention med spas Weave AI Receptionist ↗. Other practices get appointment requests for staff to review Weave AI Receptionist ↗.

UpfirstStarts at $24.95/month for 30 calls

Upfirst is our top pick for general small businesses, starting at $24.95 a month for 30 calls with a 14 day no card trial and booking into Google, Outlook, Acuity and Square Upfirst pricing ↗. For med spas, BAAs are listed only on its Custom plan, which rules it out for calls that touch health details Upfirst pricing ↗.
